Sanitation *definitely* played a part. As someone else said, "it's a bit more complex", but there is no doubt that clean water, getting sewage out of the streets, the ability to take a shower more than once a month, getting rid of lice, rats and other vermin etc played a huge roll in the decline of disease.
If we are to look at the body as a machine as pharma pretends to, it means that we should make sure that we don't pour in oil that has sand in it and make sure the filters are cleaning out any buildup. Exactly. But in today's world they'd just invent something very expensive to treat the symptoms of the sandy oil and the dirty filters until the "car" completely broke down. I'm always amazed at the campaigns to "find a cure" for diseases. (I openly *refuse* to support them). I've never once seen a campaign to "find the cause" of disease.
